Shingles
Shingles, also known as herpes zoster, is a painful skin rash caused by reactivation of the varicella-zoster virus—the same virus that causes chickenpox. After an individual recovers from chickenpox, the virus remains dormant in the body’s nerve tissues and can reactivate later in life as shingles, particularly when the immune system is weakened. The risk of developing shingles increases significantly with age, especially in individuals over 50. Other risk factors include chronic medical conditions, stress, and immunosuppressive treatments such as chemotherapy or long-term steroid use. Shingles is not directly spread from person to person, but someone with shingles can transmit the varicella-zoster virus to individuals who have never had chickenpox or the vaccine, potentially causing chickenpox rather than shingles.
Symptoms
Shingles typically begins with tingling, itching, or pain on one side of the body or face, followed by a distinctive rash that develops into fluid-filled blisters. These blisters usually scab over within 7 to 10 days and fully heal within 2 to 4 weeks. The pain associated with shingles can range from mild to severe and, in some cases, may persist long after the rash has healed—a condition known as postherpetic neuralgia (PHN). This long-term nerve pain is more common in older adults and can significantly impact quality of life. Other symptoms may include fever, headache, sensitivity to light, and fatigue. Recognising the early signs of shingles and seeking prompt medical attention can help reduce the severity and duration of the infection.
Vaccination
Vaccination is the most effective way to reduce the risk of developing shingles and its complications. In the UK, the recombinant shingles vaccine Shingrix® is recommended for adults aged 50 and over, and for younger individuals with weakened immune systems. Shingrix is a non-live, adjuvanted vaccine that offers over 90% protection and provides long-lasting immunity. The vaccine is given as two intramuscular injections, spaced 2 to 6 months apart. Unlike the older live vaccine (Zostavax®), Shingrix is suitable for immunocompromised patients and has become the preferred option for most people.
